Our website host, Squarespace, collects this through cookies and similar tools to keep the site running and to show us how many people visit. We do not use this information to identify you.

Email and phone. Some buttons open your own email program or phone. If you email us, we receive your email address and whatever you write. Email is not a secure way to send medical information, so please don't include personal health details. Call us at (614) 866-9134 instead.

Your medical information. This website does not collect or store medical records. Information you share as a patient of Canyon Eye Associates is protected under the practice's Notice of Privacy Practices, which you can request at the office.
